Failure Mode & Effect Analysis (PFMEA)

Your Motivation

  • Unexpected product defects and rejects not detected despite process design review
  • Process controls do not appear to be effective as product defects reach final inspection and the customer
  • Process controls do not prevent defects that affect quality
  • Unexpected process failures due to risks not identified during process design review

General Learning Objectives

  • Identify and prevent all potential errors in a process
  • Prioritize preventive measures based on the actual risks
  • Develop a process control plan based on the resulting potential risks

Main Focus

  • Introduction: FMEA definition, types of FMEA. APQP cycle, milestones
  • Inputs, roadmap and results: AIAG-VDA FMEA, suggested form, input data and deliverables, FMEA roadmap
  • FMEA team: teamwork, team composition, role of the team leader, team roles and responsibilities
  • Input data: Project scope, process description, flowcharts, step-by-step guidance, process functions
  • Risk analysis: failure modes, effects, severity values, root causes, probability of occurrence, current controls, prevention and detection, evidence
  • Risk management: prioritization of measures, preventive measures, detection, definition of controls
    Definition of the control plan
